We start by reviewing your hospitalization, discharge instructions, medications, diagnoses, test results, and recommended follow-up.

Our goal is to make sure you understand what happened, what requires attention now, and what questions still need answers.

Serious medical events can quickly create a complicated network of specialists and services.

BetterLife can help coordinate appointments with specialists, diagnostic testing, imaging, rehabilitation, physical or occupational therapy, home health services, pharmacies, and other providers involved in your recovery.

We help make sure the right people are involved—and that important follow-up doesn't fall through the cracks.

Review Your Medications

Hospitalizations frequently result in medications being added, stopped, or changed.

We review your medication list, help identify potential confusion or duplication, and work with your healthcare providers and pharmacy to help ensure you understand what you should be taking and why.

Your BetterLife team helps you navigate questions and challenges that arise after discharge and works with your existing providers to keep your care coordinated as your needs change.

Depending on your circumstances, that may include reviewing cardiovascular and metabolic risk, medications, blood pressure, cholesterol, diabetes risk, sleep, nutrition, exercise, mental health, existing chronic conditions, family history, and previous diagnostic testing.

We then help you understand where the greatest risks and opportunities may be and develop a clearer plan for the future.

The goal isn't simply to recover from what happened.

It's to use what happened to make better decisions about what comes next.

Rather than leaving you or your family to chase every loose end, BetterLife helps keep the process moving.

Once the immediate recovery is underway, we look beyond the hospitalization itself.

What risks contributed to the event? What hasn't been evaluated? What could be better managed? What should your healthcare priorities be now?

Post-Acute Care does not require an annual BetterLife membership.

Once your immediate recovery and health reassessment are complete, we'll help you understand your options for ongoing care—whether that's returning to your existing physicians, using BetterLife when needed, or choosing a more comprehensive ongoing relationship.

Who it’s for

Who Is Post-Acute Care For?

Post-Acute Care may be appropriate for people recovering from serious medical events or hospitalizations such as:

  • Stroke or transient ischemic attack
  • Heart attack or other major cardiovascular event
  • Major surgery
  • Serious infection or sepsis
  • Complicated hospitalization
  • New or significantly worsened chronic disease
  • Significant injury or trauma
  • A hospitalization resulting in multiple new medications or specialists
  • A medical event that has left you or your family unsure what to do next

You don't need to already be a BetterLife member.

Post-Acute Care can be accessed independently, without an annual Concierge Preventative Care commitment.
